Before Your Hike: Waiver & Medical Form

Before you head out on a guided hike with LifeTrail, please complete our short Participant Waiver, Booking Conditions and Medical Form. It only takes a few minutes and can be done on your phone or computer.

The medical questions help us keep you safe on the trail and give emergency services accurate information if it’s ever needed. The waiver covers what you’re agreeing to when you take part in an outdoor activity in a natural environment.

Please complete a new form for each hike you book, even if you’ve hiked with us before — it confirms your details and the dates for this trip.

If the participant is under 18, a parent or legal guardian needs to complete and sign the form on their behalf.

This form is for hiking activities only. It is separate from your LifeTrail Mentoring Service Agreement.

Your information is used only for your safety and for emergencies, and is handled in line with our Privacy Policy.
